CARE Insurance Change Request Form

Occasionally, individuals have a change in their insurance coverage, i.e. carrier or premium rate, during the course of a grant calendar year. If such a change occurs and per the certification and acceptance of the CARE grant, the grantee must complete an insurance change request form as soon as the grantee becomes aware of the impending change.

All insurance change forms are reviewed and considered. The NGCF will notify you via email if the requested change has been approved.
